COVID-19 vaccine is now available for everyone 16 and older. To be eligible, individuals must be a resident of the state of Kansas. The Moderna and Johnson & Johnson vaccines are not approved for those under 18.
Fill out the intake questionnaire. This questionnaire asks for your identification information, occupational info, and some health history questions. It takes about 5 minutes to fill out. The appointment time you selected is not locked in until you have finished the registration process and you have received confirmation. This is a known limitation of our system. Due to the limited amount of vaccine, appointment times can be filled quickly and you may not receive an appointment, even though you selected a time and filled out the form. This could be extremely frustrating. Please be patient.
A confirmation email will be sent after you have completed registration. You may need to check your spam/junk folder.
At the vaccine clinic: Please bring your ID / Driver's License or Birth Certificate to verify your birth day, or work ID or pay stub to verify employment.
If you receive Moderna or Pfizer vaccine, you will need to receive a second booster dose. Please make sure your schedule allows for it. You will be scheduled for your second dose at the clinic when you receive your primary dose.
