We are hosting vaccination clinics in Emporia. These clinics are for everyone 6 months and older. We will be offering Moderna, Pfizer and Johnson & Johnson vaccines, for both first dose and booster doses.
The vaccine clinics will be:
Fill out the intake questionnaire. This questionnaire asks for your identification information, occupational info, and some health history questions. It takes about 5 minutes to fill out. The appointment time you selected is not locked in until you have finished the registration process and you have received confirmation.
A confirmation email will be sent after you have completed registration. You may need to check your spam/junk folder.
At the vaccine clinic: Please bring your ID / Driver's License or Birth Certificate to verify your birth day, or work ID or pay stub to verify employment.
If you receive Moderna or Pfizer vaccine, you will need to receive a second booster dose. Please make sure your schedule allows for it. You will be scheduled for your second dose at the clinic when you receive your primary dose.
